Our team is made up of individuals, social service organizations, educational institutions, and representatives from the business community and government.
Funded partially by the Putnam County Community Foundation, PutCo 360 was launched in early 2019. Seeing a need for enhanced collaborative work between providers in our Community to increase the Opportunity Outcomes for our neighbors, Family Support Services of West Central Indiana and Cummins Behavorial Health adapted our System of Care model to fit this newly enhanced model.
To create a community of opportunity in which there are available pathways for all residents that lead to positive outcomes, including housing stability, education, health and well-being, economic security and mobility.
